The ball position is the decision, and it changes which muscles do the work. Thumb-operated trackballs keep the hand in a fixed, supported position and move the cursor with one digit, which is why they are the usual recommendation for wrist pain: the forearm never travels. The tradeoff is that fine positioning comes from the least dexterous finger you have, and thumb designs are almost always sculpted for the right hand only, so left-handed users are largely locked out. Finger-operated or centre-ball designs use the index and middle fingers, give better precision for detailed work, and are usually symmetrical, but the hand sits flatter and the thumb ends up doing the clicking. Try to match the shell to your hand size rather than buying on features, since a trackball is used with the palm parked in one place and a shell too small for a large hand cramps within an hour. A hinged or tilting base that sets the body at around twenty degrees is worth having because it rotates the forearm toward neutral. Check how the ball is supported, as designs on small bearings roll more smoothly than those on plastic pads and both need the ball lifted out and wiped every few weeks. Then decide on power: replaceable AA batteries run for months, rechargeable cells mean a cable now and then. Multi-device pairing across two or three machines is genuinely useful.
